Job Summary: The Grants Manager is responsible for the comprehensive management, compliance, and reporting of grant-funded programs. This role plays a critical part in sustaining and growing the agency's funding base by ensuring programmatic integrity, accurate fiscal tracking, and timely reporting grants.
Key Responsibilities:
Grant Management and Portfolio:
• Serve as the primary point of contact for all Ryan White Part B grant activities.
• Ensure funded services are delivered in accordance with HRSA guidelines, federal regulations, and grant award terms and conditions.
• Coordinate with staff to ensure allowable fund usage and alignment with approved scope of work.
• Prepare and submit all required reports such as performance measure data and annual reports.
• Monitor expenditures to ensure fiscal compliance and timely reimbursement request submission.
• Support the administration of all agency grants, including tracking deadlines, deliverables, and reporting requirements.
• Maintain a grants calendar reflecting deadlines, reporting periods, and renewal timelines.
• Assist staff in interpreting grant requirements and implementing grant-compliant activities.
• Review grant budgets and expenditures to identify variances and ensure appropriate cost.
Compliance and Documentation:
• Ensure programmatic compliance with applicable federal, state, and local regulations.
• Develop and maintain grant files documentation system and internal controls system.
• Coordinate with staff to ensure accurate and timely completion of audits and site visits.
• Maintain compliance with all confidentiality requirements, including HIPAA and applicable grant-specific data privacy regulations.
Data, Reporting, and Performance Management:
• Collect, analyze, and report program data in alignment with funder performance measures and organizational goals.
• Oversee data entry and quality assurance processes of all grant-related data.
• Prepare internal performance dashboards and reports for leadership review.
Grants Development Support:
• Assist in identifying new grant funding opportunities aligned with organizational priorities.
• Contribute to the development of grant applications, project narratives, budgets, and work plans.
Qualifications:
• Bachelor's degree in Public Administration, Public Health, Nonprofit Management, Business Administration, or a related field required.
• Minimum of one (1) year of experience in grants management, program compliance, or a related role. Prior experience with government-funded programs strongly preferred.
• Strong knowledge of federal grant regulations, including Uniform Guidance (2 CFR Part 200). Demonstrated ability to manage multiple projects simultaneously. Excellent analytical, organizational, and written communication skills. Proficiency in Microsoft Office Suite.
Working Conditions:
• In-person, office-based position with standard business hours.
Periodic travel may be required for funder meetings, conferences, or site visits

Outreach Specialist

Job Summary: The Outreach Specialist is responsible for conducting general prevention outreach to promote sexual health, reduce stigma, and improve engagement among populations in need and targeted HIV outreach, education, and linkage activities in accordance with Ryan White HIV/AIDS Program standards and best practices.
Key Responsibilities:
Targeted HIV Outreach and Engagement:
- Conduct targeted HIV outreach activities in priority communities consistent with Ryan White Part B guidelines and local epidemiologic data.
- Identify and engage populations in need, including individuals at increased risk for HIV or those who are out of care or newly diagnosed.
HIV Prevention and Education:
- Deliver community-based HIV prevention education, including information on condom use, Pre-Exposure Prophylaxis (PrEP), Post-Exposure Prophylaxis (PEP), and routine testing.
- Distribute prevention materials and educational resources in accordance with program policies.
- Support stigma-reduction efforts through culturally responsive education and outreach activities.
Linkage to Care and Support Services:
- Facilitate referrals and warm handoffs to medical and supportive services as appropriate.
- Coordinate with internal staff and external partners to ensure timely linkage to services.
- Assist clients with navigating healthcare and support systems, understanding available resources and eligibility requirements.
Data Collection and Reporting:
- Accurately document activities, encounters, and referrals in required systems and reports.
- Collect required outreach and performance data in compliance with reporting requirements.
- Participate in quality improvement activities related to outreach effectiveness and client engagement.
Community Collaboration and Representation:
- Represent Afya at community events, health fairs, and coalition meetings as assigned.
- Maintain positive working relationships with community partners, healthcare providers, and local organizations.
- Support agency-wide prevention and public health initiatives as needed.
Qualifications:
- High school diploma or GED required;
- Knowledge of HIV prevention, testing, and care concepts, or willingness to obtain training.
- Strong interpersonal, communication, and engagement skills.
- Ability to work flexible hours, including evenings or weekends, as needed for outreach activities.
- Valid driver’s license and reliable transportation.
Working Conditions:
- This position involves frequent community-based work in non-traditional settings.
- May require standing for extended periods, walking, and lifting outreach materials.
- Travel within the service area is required.
- Work schedule may vary based on outreach events and community needs.
